POSITION TITLE: Transition Facilitator

ORGANIZATIONAL STRUCTURE:

Consults With: Forum Staff, Principals, Teachers, Counselors, Associates, Keystone AEA staff, Parents, Students and Community Agencies
POSITION DESCRIPTION:
The Transition Facilitator is responsible for coordinating various activities related to planning and implementing the transition of youth with disabilities from school to all phases of adult life as required by law.
POSITION QUALIFICATIONS:
Must have a minimum of a BA/BS degree in a field related to special education, rehabilitation counseling, or other relevant profession that emphasizes services to individuals with disabilities.
Experience in working with organizations that provide services to individuals with disabilities is recommended.
Experience in helping youth through the transition process from the school environment to the adult living and working environment is recommended.
RESPONSIBILITIES:
TRANSITION:
Assist students receiving special education support & services (ages 3-21) and his/her family in learning about community agencies, supports, and post-secondary options.
To keep updated on community agency supports and policies, to properly inform teachers and families of eligibility criteria and supports available.
Complete Age of Majority letters to parents/guardians and students.
Maintain communication with colleges & agencies for appropriate transition information for students.
Provide Professional Development and coach with teachers around all aspects of transition planning and IDEA-DA.
TRANSITION PROGRAMS:
Monitor the Summit Program: provide professional development, linkages with agencies, contracts with agencies, attend IEP meetings, observe monthly budget and assist in staffing.
Monitor the HEART Program: assist with student support/services, communication with home school team for transitional areas, attend IEP meetings, be the district liaison for Four Mounds, NICC, Dupaco and the City of Dubuque, maintain the HEART contract with Four Mounds, coordinate HEART Advisory meetings and assist in staffing.
Monitor the Transition Alliance Program (TAP): provide oversight of the federal requirements for the program, staff development and oversight of the financial obligations. Assist team with quarterly reports, state monitoring visits, summer camp/events, assist in staffing, conduct observations and participate in the evaluation of staff.
Monitor the School-To-Work (STW) program: Provide professional development to Work Experience Coordinators (WEC) and Job Coaches on current child labor laws and safety regulations.
Collaboration with WEC for student Training Agreements and evaluation/assessment rubrics.
VERTEX:
Attend ICAP and DCAP meetings and support building implementation
Assist with database of students and businesses within Xello Work Based Learning (WBL)
Assist parents, students, WEC, Social Workers, special education teachers and others: providing contracts, identifying resources, and assuring appropriate training and instruction.
Maintain up to date records, forms and procedures as deemed necessary by district, state and federal laws.
Partner with schools to support the Vertex Initiative
Assist with WBL events, database, and collaboration with partners
Support implementation of Xello
ALTERNATE ASSESSMENT:
District liaison on the state Significant Disability Leadership Team
A. Attends state training courses and meetings
B. Plan and deliver professional development for DLM and ELAA and provide 1:1 support with teachers as needed
C. Develop and maintain databases for district and state use
MOBILITY OPPORTUNITIES VIA EDUCATION (MOVE):
Assist MOVE Trainers (MIT) with required model site paperwork, professional development, and maintenance of databases.
Oversee the MOVE equipment inventory system and work within the Destiny system and library staff.
Work with IEP teams to transfer equipment and order new equipment.
REPORTS/DATABASES/IEP/CONTACTS:
Submit reports as required by the District, AEA, Community Agencies, County and the State.
Attend IEP staffings and assist special education teams in developing an appropriate transition plan, which is part of the IEP, and which includes identification of service provider agencies, social workers, community agencies, assisting with transition assessments, as well as other pertinent information.
Contribute to diagnostic and educational teams, special education team meetings, and IEP staffings.
Maintain transition components on Sharepoint (assessments, resources, services).
Provide professional development and teacher assistance for IDEA DA compliance.
Assist with review and writing of the Local School Plans (LSP's) for each building, along with the Statement of Work (SOW).
Be the district liaison with Hills and Dales for communication procedures and intake of new students.
