Job Overview
We are seeking a compassionate and dedicated RN/ Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. This role requires a strong understanding of patient needs, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.

- 7p - 7a
- 3 shifts per week
Duties
- Administer medications and treatments as prescribed by physicians.
- Monitor patients’ vital signs and report any significant changes to the healthcare team.
- Assist with patient assessments, including physical examinations and medical history intake.
- Provide direct patient care, including wound care, catheterization, IV insertion, and tube feeding.
- Support patients with activities of daily living (ADLs) such as bathing, dressing, and mobility assistance.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Educate patients and their families on health management and disease prevention strategies.
- Participate in discharge planning and case management activities as needed.
- Uphold infection control protocols and maintain a safe environment for patients.
Qualifications
- Valid RN/ LPN license in the state of practice.
- Experience in critical care, long-term care, or assisted living preferred.
- Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is advantageous.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and CPT coding is beneficial.
- Experience working with diverse populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus.
- Strong skills in patient assessment, medication administration, phlebotomy, and basic life support (BLS).
- Ability to perform procedures such as venipuncture, suturing, or airway management as required by the role.
- Excellent communication skills and the ability to work effectively within a team environment.
