Job Summary

The Lung Health Navigator serves as a patient support and care coordination resource to help individuals successfully navigate the lung cancer screening and treatment process. The navigator works closely with hospital teams, community organizations, and statewide partners to reduce barriers to care, improve patient engagement, and support timely preventive and clinical services.

The Lung Health Navigator provides individualized assistance to patients, families, and caregivers throughout the continuum of lung health care, including preventive lung cancer screening, diagnosis, treatment coordination, survivorship, and tobacco cessation support. This position is grant-funded.

Responsibilities (essential Functions)

  • Meet with patients to assess barriers to care and provide navigation and care coordination services related to lung cancer screening, tobacco treatment, and follow-up care.
  • Document patient interactions and navigation activities in the electronic health record (EHR) and data tracking systems.
  • Work collaboratively with hospital staff, primary care providers, radiology departments, pulmonology teams, and community partners to coordinate care and streamline identifying eligible patients, ensuring a shared decision-making consult is completed, a low-dose CT scan gets scheduled and completed, and schedule annual follow-up.
  • Support implementation and sustainability of preventive lung cancer screening workflows and referral pathways.
  • Assist patients with scheduling, reminders, transportation coordination, insurance navigation, and addressing social or logistical barriers that may impact care.
  • Provide education regarding lung cancer screening guidelines, shared decision-making, tobacco cessation resources, and available community services.
  • Facilitate referrals to the Oklahoma Tobacco Helpline, American Lung Association programs, community health workers, food resources, transportation assistance, and other support services as appropriate.
  • Participate in community outreach, awareness campaigns, and educational events related to lung health, tobacco treatment, and preventive care.
  • Maintain accurate records, reports, and patient activity documentation as required by hospital leadership and grant partners.
  • Participate in the Oklahoma Lung Navigator Network and other statewide lung health coalitions to collaborate with peer screening programs, share lessons learned, discuss best practices, and support continuous program improvement.
  • Participate in meetings, trainings, and quality improvement activities related to lung health and preventive care initiatives.
  • Demonstrates knowledge and support of DRH Health’s mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
  • Actively participate in required training and professional development activities.
  • Maintains professional and technical knowledge through continuing education opportunities, including internal and external educational offerings.
  • Regular attendance and punctuality for scheduled shifts.
  • Utilization of assistive devices for lifting is mandatory.
  • Must adhere to safety protocols at all times.
  • Per DRH policy, all required conditions of employment must be met and maintained, including required vaccinations.
  • Implement DRH Standards of Behavior and exhibit behaviors consistent with DRH core values.
  • Performs other related duties as assigned.

Qualifications

Minimum Qualifications: Communication skills, including fluency in oral and written English. Basic computer skills, including the ability to send/receive/email, navigate information technology associated with the position, and use Electronic Health Record information tools. Excellent written, verbal, and interpersonal communication skills; Ability to adapt procedures, processes, tools, equipment, and techniques to accomplish the requirements of the position. Advanced fingerprint background checks are required for this position.

Works effectively in a team environment, participating and assisting their peers. Ability to organize, coordinate, and demonstrate judgment, professionalism, diplomacy, assertiveness, and initiative. Frequent departmental interfaces with internal and external customers to coordinate and provide an optimal level of program services. Knowledge and ability to develop and implement policies and procedures. This position requires effective time management skills and the ability to comply with organizational policies and operate independently or under a supervisor’s direction.

Education and/or Experience: Bachelor’s degree in a health-related field and two (2) years of experience in patient navigation, community health, care coordination, public health, or related support services; OR

Associate’s degree in a health-related field and four (4) years of related experience. RN is preferred.

Certifications, Licenses, Registrations: A current, valid driver’s license and automobile liability insurance must be maintained. Must have reliable transportation. If RN, current, unencumbered RN license in the State of Oklahoma or multistate licensure in a party state in compliance with primary residency rules and requirements.

This position requires respirator and fit testing. Required testing will be performed on all team members at risk for exposure to airborne infectious diseases, droplet producing procedures, or any caustic chemicals.
