Nurse Practitioner (Wound Care)
FullTime | Monday–Friday | No Nights, Weekends, or OnCall

What You’ll Do
Provide wound assessments, treatments, and followup care in partnered facilities
Collaborate with facility nursing staff and physicians
Document visits accurately in our streamlined EMR
Participate in careplan development and qualityimprovement initiatives
Deliver compassionate, patientcentered care to a diverse geriatric population
What We’re Looking For
Licensed Nurse Practitioner (NP) in Illinois
Strong clinical judgment and communication skills
Reliable, organized, and comfortable working independently
Passion for serving seniors and improving patient outcomes
Valid driver’s license and ability to travel locally between facilities
