Dover High School seeks a transition coordinator to join our diverse community of students and educators. The role of a special education transition coordinator is to provide students, parents, and staff with transition planning, employment skills development, transition assessments, and exposure to job/work-based opportunities for students with special needs. A primary responsibility is to coordinate with schools and local resources to provide vocational and ELO opportunities and develop post-secondary linkage.
Essential Duties and Responsibilities:
• Proven ability to work effectively with/for children and adults in a multi-cultural and diverse community.
• Empathetic to the needs of all students with disabilities who require transition services.
• Provide age-appropriate transition assessment and narrative recommendations for transition planning
• Provide special services staff with resources needed to develop and carry out effective transition plans
• Collect data from transition assessments to identify students' preferences, interests, needs, and strengths
• Prepare and maintain consistent transition activities and curriculum, lessons, and age-appropriate transition assessments in academic planning and organization, alternative academic planning, job preparation, and self-determination skills.
•Coordinate and serve as a liaison and advisor for ELO programs offered to identified students (e.g., currently IMPACCT Academy).
•Provide direct support and vocational counseling for all referred students per their case manager’s request.
•Identifies and connects special education students to Vocational Rehabilitation and is charged with processing all individual applications, ensuring intake meetings are conducted after eligibility has been determined.
•Initiates and sets up all collaboration meetings with Vocational Rehabilitation Services, working directly with VR representatives, students, families, and team members.
•Coordinates and serves as a liaison for Vocational Rehabilitation Workshops for Pre-Employment Transition (ETS) provided monthly by a Vocational Rehabilitation instructor within the school setting.
•Serves as a liaison between community transition agencies and special education teams.
• Helps facilitate the annual Transition Fair held at DHS in conjunction with the local agency.
•Attend progress monitoring, evaluation, and IEP meetings for special education students as appropriate and as required by law.
•Employs various instructional techniques and strategies consistent with the needs and capabilities of the individuals or students involved.
•Consults with the Dean of Student Services about budget items that would support students with transitioning.
•Teaches weekly Advisory classes (this takes planning with POL directives).
•Schedules students and teaches weekly FLEX classes.
•Strong communication and organizational skills required
