Position Summary
The PRN Registered Nurse provides as-needed home care nursing assessments and performs occasional skilled nursing tasks based on client needs. This role supports the agency in ensuring timely admissions, reassessments, caregiver supervision, and safe delivery of home care services.
Key Responsibilities1. Nursing Assessments (Primary Role)
- Conduct initial nursing assessments for new clients.
- Complete 90-day and 45-day reassessments, or more frequent as required.
- Update and revise the client’s Plan of Care based on assessment findings.
- Perform home safety evaluations and identify risks.
- Review medications, complete medication reconciliation, and educate clients/families.
2. Skilled Nursing Tasks (Occasional)
- Provide PRN nursing care such as:
- Wound care
- Dressing changes
- Injections
- Catheter care
- Vital signs monitoring
- Disease management (CHF, COPD, diabetes, etc.)
- Train caregivers on basic skills as needed.
3. Supervisory Visits
- Conduct RN supervisory visits to observe caregivers, ensure compliance, and provide teaching.
- Document findings and communicate with the Administrator.
4. Documentation & Communication
- Complete visit notes and care plans in accordance with state regulations and agency policy.
- Communicate changes or concerns to the care team promptly.
- Maintain HIPAA compliance at all times.
Qualifications
- Active RN license in the required state(s).
- CPR certification.
- At least 1 year of clinical experience (home care preferred, not required).
- Strong assessment and communication skills.
- Reliable transportation and ability to travel to client homes.
- Must pass background check and meet agency onboarding requirements.
