Duties
- Assist client with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques.
- Monitor patients’ vital signs and observe for any changes in health status, reporting findings accurately to healthcare professionals.
- Administer medications following prescribed care plans while maintaining compliance with HIPAA regulations to protect client confidentiality.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
- Support clients during outings or appointments by providing personal transportation assistance and ensuring safety throughout travel.
- Conduct patient observation to identify behavioral changes or signs of medical issues such as dementia or neurodevelopmental disorders like autism.
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being.
- Document all care activities accurately in client records, communicate effectively with clients and their families, and adhere to all home health standards including CPR and first aid protocols.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated knowledge of patient care techniques such as medication administration, vital signs monitoring, and patient transfer methods.
- Experience providing specialized care for individuals with Alzheimer’s disease, dementia, or memory impairments is advantageous.
- Background working with individuals with developmental disabilities or neurodevelopmental disorders like autism supports your ability to deliver personalized support.
- Familiarity with creating and following care plans aligned with residents’ needs ensures comprehensive caregiving.
- Strong communication skills for client interaction, family updates, and team collaboration are essential.
- Certification in CPR and first aid is required; additional training in hospice care or geriatrics is a plus.
