Summary:
The Residential Coordinator provides supports and training necessary to help individuals with disabilities live and work in a safe, healthy, and inclusive environment. This position coordinates daily assistance for individuals to complete personal care routines, maintain employment participation, and expand personal relationships and community involvement. The Residential Coordinator supervises Direct Support Professionals and oversees the daily operations of designated community homes.
Key Responsibilities:
- Attend and participate in Individual Plan of Service (IPOS) meetings; develop and implement formal objectives and informal training strategies.
- Participate in review meetings to update service plans and communicate all significant changes in writing to Service Coordinators.
- Lead team meetings to revise residential plans and ensure individuals and families understand program revisions.
- Ensure each individual has a current Residential Plan; review annual assessments and initiate new assessments every three years. Maintain and update information in THERAP.
- Monitor each individual’s status monthly; complete quarterly progress reports and submit to the Residential Director by the 10th of each month.
- Review and reconcile consumer banking records; ensure all receipts, deposits, and petty cash ledgers are accurate and submitted timely.
- Prepare weekly staffing schedules for assigned CTH/SLP homes; review and approve leave requests based on minimum coverage requirements.
- Review and approve weekly staff time in Paychex; ensure leave is entered and approved correctly for each pay period.
- Conduct interviews to maintain an active hiring pool; complete monthly service provision logs and submit by the 3rd of each month.
- Meet with assigned staff monthly; prepare agendas and minutes and submit to the Residential Director within five days.
- Visit each assigned residence at least once per week, including one after‑hours visit weekly.
- Review Medication Administration Records (MARs) monthly; provide feedback and monitor medication error reporting.
- Review grocery lists for menu/diet compliance and approve prior to shopping.
- Complete monthly home study checklists and report maintenance or safety concerns.
- Complete performance evaluations for assigned staff.
- Review incident/accident reports and ensure appropriate follow‑up (MD appointments, counseling referrals, etc.).
- Complete the Residential Coordinator Checklist monthly and submit to the Executive Director and Residential Director by the 30th.
- Maintain individual records and house inspection books in compliance with SCDDSN requirements; submit outdated records for storage per retention guidelines.
- Provide on‑call support for assigned residences, including after‑hours, weekends, and holidays (rotation may apply).
- Perform other duties as assigned.
Pay: $40,000.00 - $46,000.00 per year
