Peer Mentor
Peer mentors provide information, resources, guidance, and support to an individual who is a waiver recipient. The service is designed to foster connections and relationships that build individual resilience. Peer mentors encourage individuals with developmental disabilities to share their successful strategies and experiences in navigating a broad range of community resources beyond those offered through the waiver. Mentors advocate for and make a plan to achieve integrated opportunities and experiences in living, working, socializing, and staying healthy and safe in their own life, as well as to overcome personal barriers which are inhibiting them from being more independent.
ESSENTIAL DUTIES and RESPONSIBILITIES
•Have 1:1 sessions with Mentees as scheduled including sessions to discuss the plan the individual’s specific interests and desired outcomes
•Explain community services and programs and suggests strategies to the Mentee to support with achieving their desired outcomes
•Provide information from own experience to support Mentee with problem solving decision making, developing supportive community relationships, and exploring community resources to promote increased independence
•Assist Mentees in developing personal plans to access identified activities, supports, services, or resources
•Demonstrate effective and professional communication with persons served, their families/support systems, as well as other members of their team.
•Maintain accurate time by clocking in and out at the start and end of shift.
•Complete and submit required documentation
•Meet routinely with Supervisor to discuss progress made and any barriers experienced by themselves or the Mentee
Qualifications:PREFERRED EXPERIENCE
•Must meet criteria for position as outlined by DMAS regulation 12VAC-30-122-450
•Must have completed the DBHDS DD Peer Mentor Training
•Complete training as required by Dominion Care
•Willingness to learn and grow
•Desire to make a difference
