Role Description The Home Health Nurse is a full-time, on-site role based in Knoxville, TN, focused on providing pediatric nursing care in patients’ homes and related community settings. Day-to-day responsibilities include assessing pediatric patients, developing and implementing individualized care plans, administering medications and treatments, and providing wound care and ongoing monitoring. The role involves close collaboration with physicians, hospital care teams, and family members to coordinate continuity of care after hospital discharge. The Home Health Nurse educates families on disease management, home care procedures, and safety practices, while documenting all care activities in compliance with regulatory and hospital standards.
Qualifications
- Strong clinical skills in Nursing and Medicine appropriate for pediatric home health care.
- Experience in Home Care and Patient Care, with a focus on supporting children and families in the home setting.
- Competence in Wound Care, including assessment, treatment, and monitoring of pediatric wounds.
- Competence in Central Line maintenance, Port access and IV infusions.
- Active Registered Nurse (RN) license in the state of Tennessee; pediatric or home health experience preferred.
- Current Basic Life Support (BLS) and Pediatric Advanced Life Support (PALS) certifications or willingness to obtain.
- Excellent communication, documentation, and family education skills; ability to work collaboratively in interdisciplinary teams.
- Valid driver’s license, and ability to travel to patient homes within the service area.
- Ability to lift, move, and position patients as needed and to adapt to varying home environments.
