Role Description The RAI MDS role is a full-time, on-site position located in Orange, VA. This role is responsible for completing accurate and timely MDS assessments, care area assessments, and related documentation in accordance with federal, state, and facility guidelines. The RAI MDS team member collaborates closely with nursing, therapy, social services, and other interdisciplinary team members to develop, update, and evaluate individualized care plans. Daily tasks include reviewing clinical records, monitoring resident status changes, coordinating assessment schedules, and ensuring correct coding for reimbursement and quality measures. The role also involves educating staff on RAI/MDS processes, supporting survey readiness, and participating in quality improvement initiatives that enhance resident outcomes and regulatory compliance.
Qualifications
- Demonstrated expertise in RAI/MDS assessment processes, care area assessments, and care plan development.
- Strong clinical documentation and chart review skills to ensure accurate coding and compliance.
- Ability to collaborate effectively with an interdisciplinary team, including nursing, therapy, and social services.
- Solid knowledge of long-term care regulations, reimbursement guidelines, and quality measures.
- Effective communication, organization, and time-management skills to manage multiple assessments and deadlines.
- Proficiency with electronic health record (EHR) systems and basic computer applications.
- Current licensure or certification as a Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN), as required by facility and state regulations.
