Licensed Clinical Social Worker (PRN) Please Note: New Directions is the Geriatric Behavioral Health Unit for Dodge County Hospital.

HOURS: PRN.

POSITION SUMMARY:

The Social Worker is responsible for providing social service functions in the behavioral health care setting. Provides assessments, treatments, and case management to patients and families. Is a resource person to healthcare professionals concerning psycho-social problems and community resources. Maintains performance improvement activities within the department.

EDUCATION:
- Master’s degree in social work or clinical nurse specialist for psychiatric mental health
- Current licensure such as LCSW or LPC experience in psychiatric setting preferred
- Knowledge of community resources for older persons, knowledge of Medicare, Medicaid and insurance requirements for continued care
- Familiarity of medical terminology and conditions as these impact treatment and discharge needs
- Familiarity with medical team roles in hospital departments

RESPONSIBILITIES:

- Clinical Therapy responsibilities, care managements which include treatment plans for patients
- Able to collect and evaluate data for social services and discharge planning
- Therapist must maintain an accurate and up-to-date knowledge of relevant regulations and standards regarding provisions of social services while implementing treatments and discharge planning.
- Provide individual and group therapy/family group therapy
- Communicate and update families when needed
- Complete all required assessments, such as Suicide Risk Assessment and Psycho-Social Assessment by due date
- Assist on overseeing the intake of referral, and follow-up with referral sources
- Participate in the weekly treatment team meeting and ensure appropriate follow-up documentation
- Ensure activity and socialization groups are occurring per schedule
