- Monday – Thursday & Every Other Friday : 8:00 AM – 5:00 PM

- DMD or DDS from an accredited U.S. program
- Active State Dental License

- Pass a background (Criminal & Malpractice), urinalysis & applicable testing, shots, physical exam and vaccinations.
