We are looking for a mature, independent person to provide service for a 90 yr. old (F). 12 hours a day 2 days a week (Sat. & Sun) prefer 8am to 8pm. (in the North Phila. area). Early onset dementia, independent, must speak & understand fluent English. Supervise/monitoring while in the shower. Medication reminder and assist with eye drops. Daily exercise (walks), on-going interaction/communication, and cooking & light housekeeping is also required

RESPONSIBILITIES

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medication reminders
• Engage patients in exercises or other activities

QUALIFICATIONS

• Graduated from an accredited Home Health Aide program
• High School Diploma or GED
• One-year prior professional experience
• Driver’s license required
• CPR certification preferred
