Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor and observe patient health status by recording vital signs and reporting any changes to healthcare professionals promptly.
- Provide medication administration and ensure proper documentation in electronic health record (EHR) systems while maintaining strict compliance with HIPAA regulations.
- Support patients with specialized needs including Alzheimer’s care, dementia care, hospice care, and working with individuals with developmental disabilities or disabilities.
- Prepare nutritious meals and assist with feeding, ensuring dietary restrictions and preferences are respected.
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being.
- Document medical records accurately and assist healthcare teams by updating medical documentation regularly.
Skills
- Proven experience in home health or long-term care environments, including assisted living or nursing home settings.
- Strong patient observation skills to detect changes in condition or behavior promptly.
- Knowledge of medical terminology, medication administration protocols, and vital signs measurement.
- Familiarity with EMR (Electronic Medical Records) and EHR systems for accurate documentation.
- Ability to work independently while following detailed care plans and instructions from healthcare providers.
- Compassionate caregiving skills with experience supporting seniors and individuals with disabilities or memory impairments.
