MDS Nurse - Sylvania, OH

We are seeking a skilled and compassionate MDS Nurse to coordinate our MDS and Care Planning process. As a vital member of our Interdisciplinary team, you will be responsible for ensuring timely and accurate submission of MDS assessments to the State agency.
Responsibilities:
Assist with nursing staff education in the documentation, MDS, CAA's, and Care Plan process
Accurately complete MDS assessments in accordance with established time frames for new admissions, quarterly, annual, and significant changes
Precheck and electronically submit MDS's to the State agency, review validation reports, and make necessary changes
Coordinate the scheduling and timely completion of MDS's and Care Plans with the Case Manager and Interdisciplinary team
Participate in the QAPI process
Electronically access, print, and evaluate Quality Indicators
Requirements:
Graduation from an accredited school of Nursing
Current license as a Registered Nurse with the State Board of Nursing
CPR certification recommended
Ability to work in a fast-paced environment with attention to detail and excellent communication skills
