Medical Social Worker (MSW or LCSW Required)

Overview
We are seeking a compassionate and dedicated Licensed Clinical Social Worker (MSW or LCSW) to join our team. The ideal candidate will have a strong background in providing therapeutic support to individuals facing various health challenges. This role essential for the delivery of varied social work services to hospice patients and families, including initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, and bereavement services.

Responsibilities

• Conduct thorough patient assessments to identify needs and develop personalized treatment plans.
• Provide individual and group therapy sessions and clinical counseling.
• Collaborate with other healthcare professionals to coordinate comprehensive home or community care for clients.
• Maintain accurate and confidential client records in compliance with ethical standards and legal requirements.
• Stay updated on best practices in psychopharmacology to provide informed recommendations regarding medication management.
• Engage in ongoing professional development to enhance therapeutic skills and knowledge.

Qualifications

• Master's Degree of Social Work
• Certified Social Work Associate (MSW or CSWA), in the state of Michigan
• A demonstrated ability in casework, counseling and group work, including clinical social work experience preferably in a healthcare or hospice setting.
• Basic computer knowledge.
• Possess valid state driver’s license and reliable automobile and be willing to operate personal car as necessitated by nature of job.
• Proof of current automobile insurance with minimum state financial responsibility limits
