Overview
We are seeking a dedicated and compassionate full-time Case Manager position at our foster care facility. The ideal candidate will play a crucial role in supporting individuals facing various challenges, including addiction, mental health issues, and housing instability. The Case Manager will work collaboratively with clients to develop personalized plans that promote recovery, stability, and overall well-being. This position requires strong interpersonal skills, a commitment to client advocacy, and the ability to navigate complex systems.
The ideal candidate will be responsible for managing a caseload of youth in care, coordinating services and supports, maintaining accurate documentation, and collaborating closely with families, social workers, and service providers to ensure the best outcomes for each child. Duties
Case Manager (this position may, if approved by HHSC-Child Care Regulation Division, serve as the Licensed Child Care Administrator for the operation) that is responsible for the overall administration, operations, and management of services.
· Completing an admission assessment or any other evaluation of a child for placement.
· Under the supervision of the Administrator and Executive Director, the employee provides administrative staff support related to planning and coordinating the daily departmental activities and performs related work as required.
· Quality Improvement (CQI) is a process that can help foster care facilities improve the quality of their services.
· Collaborate and research IT Systems for T3C. We are responsible for finding and researching 2 IT Systems and providing the cost, pros, and cons.
MINOR DUTIES AND RESPONSIBILITIES
§ Crisis Management de-escalation techniques, crisis lines, and crisis stabilization services
Ø Provide crisis counseling and emotional support to clients, utilizing active listening and empathy to establish rapport and build trust. Monitor and evaluate clients' progress throughout the crisis intervention process, adjusting plans and services as necessary.
Ø Assesses children and their sponsors, develops service plans, and provides crisis intervention when needed. This may include involving law enforcement, Child Protective Services, or other stakeholders.
Ø Provides crisis mental health services in a variety of settings, including foster homes, short-term crisis residential services, and urgent care centers. They may also work with law enforcement, social services, or legal services.
Ø Develops and maintains a crisis response plan, including roles, communication protocols, and training exercises. They also manage resources, monitor threats, and ensure the plan is up to date.
§ A "Behavior Support/Mentor" role in a foster care facility involves actively building relationships with foster children, providing consistent emotional support, implementing positive behavior management strategies, and guiding them in developing life skills to address behavioral challenges, all while acting as a positive role model and advocate for their well-being within the foster care system. Assessing needs: Evaluate the psychological and physical needs of children in foster care.
Ø Building relationships:
Establishing a trusting rapport with foster children through regular interactions, active listening, and demonstrating genuine care to understand their individual needs and challenges. Developing plans: Create strategies to help children develop healthy attachments with caregivers and improve their continuity of care.
Ø Behavior management:
Implementing evidence-based positive behavior support plans, including identifying triggers for challenging behaviors, teaching coping mechanisms, and providing consistent consequences for appropriate actions.
Ø Life skills development:
Assisting children in acquiring essential life skills like problem-solving, communication, self-regulation, time management, and social skills through role-playing, coaching, and practical application. Providing backup services: Provide backup services to other units and areas as needed.
Ø Individualized support:
Creating and implementing personalized behavior plans based on each child's specific needs, including regular progress monitoring and adjustments as necessary.
Ø Collaboration:
Working closely with foster parents, caseworkers, therapists, and other professionals involved in the child's care to coordinate support and ensure consistency across different settings.
Ø Trauma-informed care:
Understanding the impact of trauma on children's behavior and utilizing trauma-sensitive approaches in interactions and interventions.
Ø Advocacy:
Representing the child's needs and concerns to relevant parties, including advocating for appropriate services and supporting their participation in decision-making processes.
Ø Documentation:
Maintaining accurate records of behavioral observations, interventions implemented, progress made, and any significant incidents, adhering to agency policies and reporting requirements.
§ Aftercare Case Manager in a foster care facility provides support and services to children and families after a child has returned home from foster care. Their responsibilities include:
Ø Developing aftercare plans: Working with families to create plans for a smooth transition and continued support
Ø Providing resources: Ensuring that children and families have access to the services and support they need
Ø Monitoring progress: Regularly checking in with families to assess progress and home safety
Ø Engaging the community: Working with the community to meet the needs of children and families
Ø Completing discharge summaries: Creating summaries for youth who are being discharged or transferred from care
Qualifications
- Bachelor’s degree in Social Work, Psychology, or a related field is preferred.
- Experience in addiction counseling or behavioral health is highly desirable.
- Strong crisis management skills with the ability to remain calm under pressure.
- Excellent communication skills, both verbal and written, for effective client interaction.
- Proven ability to work collaboratively within a multidisciplinary team environment.
- Knowledge of workforce development initiatives is beneficial.
