Job Summary
The Day Services 1:1 Team Member Assistant provides individual support to assigned team members that consists of recreational, educational and social activities in the company facility and in the community. This is a part-time job, Monday-Friday from 9am-1pm
Job Duties and Responsibilities:

· Develop and implement in house and community-based social, educational, recreational and daily living skills training to individuals with varying abilities as outlined in the Person-Centered Plan. Remain current in and implement new methods of day services training and community-based education of team members.
· Provide 1:1 and small group support to train and assist team members in day services, ensuring the highest quality standards are being met.
· Respond to team member needs, conflicts, challenging behavior, and any other inappropriate behavior following the organization's principles and strategies for team member interaction.
· Assist team members with activities of daily living (ADLs) including toileting and other personal care activities that are essential to caring for oneself. Use equipment and tools such as a hoyer lift and gait belt to safely move consumers from one location to another.
· Access team members’ progress and make recommendations to best support team members.
· Record pertinent information as necessary to include team member case notes and tracking of individual goals and objectives.
· Transport team members safely to and from day services activities using company provided vehicles, as necessary.
· Work cooperatively with all staff to achieve department and Company goals.
· Maintain confidentiality policies of the corporation.
· Participate in the Real-Time Feedback and Goal-Achievement Coaching Program at Diverse Options, Inc.
· Maintain day services areas, corresponding equipment and company vehicles in a clean, orderly and safe manner.
· Serve as a back-up for other site staff, as needed.
· Other duties as assigned.
Job Requirements:
· Demonstrated ability to communicate in a professional manner so others will understand; excellent written and verbal communication skills.
· Proficiently skilled at active listening; Gives full attention to what other people are saying, takes time to understand the points being made, and asks questions without interrupting at inappropriate times.
· Demonstrated ability to work with individuals with challenging behaviors.
· Demonstrated ability to assist individuals with feeding.
· Demonstrated ability to respond to situations in an honest and ethical manner and to maintain a high level of confidentiality when necessary.
· Demonstrated ability to multi-task, prioritize and manage time effectively.
· Demonstrated ability to use logic and reasoning in decision-making to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
· Knowledge of how to copy and print documents using a multi-functional printer.
· Knowledge of how to transmit information or documents to customers, using computer, mail, or fax.
· Proficiently skilled with basic computer applications including Microsoft Office products.
· Knowledge of good housekeeping practices.
· Demonstrated ability to operate light medical equipment, including a hoyer lift and sit to stand.
Education & Experience Required:
· High school diploma or equivalent.
· Two (2) years of directly related experience.
Education & Experience Preferred:
Bachelor's degree in: Psychology, Social work or Special Education. Or completion of training course(s) on supporting individuals with special needs.
· One (1) year of experience supporting individuals of varying abilities in a group or nursing home environment.
Additional Requirements:
At the time of hire, must submit to and pass a caregiver background check, as required by The Caregiver Law under Wis. Stat. § 50.065.
Physical Requirements:
· Ability to balance, stretch, crouch, bend, stoop, crawl, walk, kneel, twist, climb, stand, and sit.
· Ability to lift and carry up to 50 lbs.
· Ability to push and pull up to 200 lbs.
Driving Requirements:
· If individual will be driving his/her own personal vehicle for business purposes, proof of adequate personal auto insurance is required.
· If individual will be driving a Diverse Options, Inc. vehicle, he/she must meet Company driving standards, which include the following:
o Must be at least 21 years old,
o Must have a current valid driver’s license, and
o Must have a good driving record.
