Duties
- Assess individual transportation needs and develop personalized plans to facilitate access to healthcare appointments, social activities, and community resources.
- Collaborate with local transportation providers, social service agencies, and community organizations to coordinate services effectively.
- Utilize motivational interviewing techniques to engage clients, build trust, and encourage positive behavioral change.
- Provide crisis management support when clients face urgent transportation or social service barriers, ensuring immediate safety and connection to appropriate resources.
- Conduct outreach activities to educate community members about available transportation options, workforce development programs, and behavioral health services.
- Maintain detailed records of client interactions, service plans, and system referrals while ensuring confidentiality and compliance with privacy standards.
- Advocate for clients within the community system by connecting them with addiction counseling, behavioral health support, or social work services as needed.
Experience
- Proven experience in workforce development, social work, or community outreach roles that involve coordinating multiple service providers.
- Knowledge of Section 8 housing programs and familiarity with public transportation systems.
- Background in addiction counseling or behavioral health services is highly desirable.
- Strong skills in motivational interviewing and crisis management to effectively support clients facing complex challenges.
- Experience working with diverse populations in a community setting; sensitivity to cultural differences is essential.
- Excellent communication skills with the ability to motivate individuals towards positive outcomes while maintaining professionalism under pressure.
