Licensed Master Social Worker (LMSW) to provide clinical social work services in an inpatient medical-surgical or rehabilitation setting. The LMSW will deliver psychosocial assessments, discharge planning, patient/family counseling, and interdisciplinary care coordination to support hospitalized patients.
- Conduct psychosocial assessments for inpatient med/surg or rehab patients
- Develop and implement patient-centered care and discharge plans
- Provide crisis intervention, supportive counseling, and patient/family education
- Collaborate with physicians, nurses, and interdisciplinary teams
- Ensure compliance with policies, regulatory guidelines, and NYS social work standards
- Document assessments, care plans, and interventions in the EMR
- Coordinate community resource referrals
- Facilitate safe discharge planning to rehab, SNF, home care or community services
- Assist with insurance, benefits, and entitlement programs as needed
- Master’s Degree in Social Work (MSW) – Required
- Active Licensed Master Social Worker (LMSW) issued by NYSED (Primary Source Verified)
- Minimum 1 year of LMSW experience
- Hospital experience preferred, especially inpatient med/surg or rehab
