WHO WE ARE
Diverse Care Clinic is an independent primary care practice in Sherman, TX, owned and operated by Nidish Jose, AGNP-C — a board-certified Adult-Gerontology Nurse Practitioner committed to building something different in North Texas healthcare. We provide comprehensive adult primary care including chronic disease management, preventive wellness, Medicare Annual Wellness Visits, and specialty services for a diverse adult patient population in Grayson County and beyond. We are expanding our clinical team and are looking for the right person to grow with us.
THE OPPORTUNITY
This is a full-time, in-person NP position at our Sherman clinic. You will work alongside the Owner/Clinical Director in a collaborative, two-provider clinical model with a dedicated MA and administrative support team. This is a genuine opportunity to practice medicine at the full scope of your Texas APRN license in an independent, practitioner-owned setting — without the constraints of hospital system protocols, without overnight call, and without a productivity treadmill.
We are a clinic that has been intentionally rebuilt from the ground up with clinical quality, patient experience, and operational integrity as our foundations. WHAT YOU WILL DO
- Conduct comprehensive history and physical examinations for new and established adult patients across a full primary care scope
- Diagnose and manage acute and chronic conditions including hypertension, diabetes, hyperlipidemia, thyroid disease, COPD, depression, and anxiety
- Perform and interpret preventive care including age-appropriate screenings and Medicare Annual Wellness Visits (G0438/G0439)
- Provide evidence-based care for patients managing complex or chronic conditions, including those requiring specialty medication management
- Participate in integrated care for patients with substance use and behavioral health needs alongside primary care — non-judgmental, protocol-driven, and fully supported by the clinic's existing clinical framework
- Perform and interpret CLIA waived point-of-care testing in the clinic setting
- Complete all clinical documentation in Tebra/Kareo EHR same day as encounter — SOAP format, E/M compliant
- Collaborate with the clinical and administrative team on quality improvement, care coordination, and patient follow-up
- Participate in chronic care management (CCM) and care coordination for Medicare patients with multiple chronic conditions
WHAT WE ARE LOOKING FOR
Required:
- Active, unrestricted Texas APRN license — Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred
- Current national board certification (FNP-C, AGNP-C, or equivalent)
- Active DEA registration, or eligibility and willingness to obtain DEA registration upon hire
- Current BLS/CPR certification
- Strong clinical documentation skills — commitment to same-day, complete, E/M-compliant charting
- Must be authorized to work in the United States — we do not sponsor work visas
Preferred:
- 3 years of experience in primary care, family medicine, urgent care, or internal medicine
- Experience managing patients with complex chronic conditions or behavioral health comorbidities
- Experience with EHR documentation — Tebra/Kareo experience is a strong plus
- Medicare billing experience including AWV coding (G0438/G0439) and CCM (CPT 99490)
- Bilingual English/Spanish — meaningful asset for serving Grayson County's patient population
Pay: $45.00 - $117,739.80 per hour
