Position Overview
Ditmas Park Nursing & Rehab is seeking an experienced and compassionate Dietitian to oversee the nutritional care of our geriatric residents.
The Dietitian will assess residents’ nutritional needs, develop individualized care plans, monitor clinical outcomes, and collaborate closely with nursing, medical, dietary, and rehabilitation staff. Key Responsibilities
Complete comprehensive nutritional assessments for new admissions and current residents
Develop and update individualized nutrition care plans
Monitor residents’ weight, nutritional status, laboratory results, food intake, and clinical progress
Identify residents who may be at risk for malnutrition, dehydration, weight loss, or other nutrition-related concerns
Provide recommendations for therapeutic diets, supplements, and nutrition interventions
Participate in interdisciplinary care plan meetings
Collaborate with physicians, nurses, speech therapists, rehabilitation staff, and the dietary department
Educate residents, families, and staff regarding nutritional needs and dietary recommendations
Maintain accurate and timely documentation in residents’ medical records
Support compliance with federal, state, and facility dietary regulations
Assist with survey preparation and ongoing quality improvement initiatives
Qualifications
At least one year of healthcare dietary or clinical nutrition experience
Skilled nursing, rehabilitation, long-term care, or geriatric experience strongly preferred
Strong knowledge of nutritional assessments, therapeutic diets, and care planning
Ability to communicate effectively with residents, families, and clinical staff
Strong organizational and documentation skills
Ability to work independently while contributing to an interdisciplinary team
Dietitian licensure or registration is preferred but not required; relevant experience will be strongly considered
Compensation and Benefits
Competitive compensation of $40.00–$45.00 per hour
