Diabetes Educator / Registered Dietitian (CDCES Preferred)

We are seeking a motivated Dietitian or Diabetes Educator to provide Diabetes Self-Management Education and Support (DSMES) within an established outpatient medical practice. This role focuses on empowering patients through personalized nutrition counseling, glucose management education, and sustainable lifestyle modifications to improve long-term health outcomes.

Responsibilities
- Provide Diabetes Self-Management Education and Support (DSMES)
- Conduct comprehensive patient assessments, including dietary history and lifestyle evaluation
- Develop individualized care plans incorporating nutrition strategies, glucose monitoring, and lifestyle interventions
- Educate patients on blood glucose control, carbohydrate counting, medication adherence, and nutrition planning
- Document visits and track patient progress using the EMR system
- Collaborate with referring providers to support coordinated patient care
Qualifications
- Registered Dietitian (RD/RDN) or other qualified clinician eligible to provide diabetes education services
- CDCES certification preferred (or willingness to obtain)
- Experience in diabetes education, nutrition counseling, or chronic disease management
- Comfortable using EMR systems and documenting patient encounters
- Strong patient education and health coaching skills to support long-term lifestyle changes
