POSITION SUMMARY:
- Responsible for the efficient and accurate coordination of home health visits for patients
- Maintain professional, open communication with clinicians, patients, and caregivers regarding visit schedules
- Coordinate closely with intake, clinical management, & billing teams to ensure smooth transitions & accurate visit tracking
- Maintain compliance with Medicare and agency-specific visit documentation and frequency rules
- Monitor & report scheduling metrics & participate in quality initiatives to enhance efficiency and customer satisfaction.
- Th HH Scheduler position is an in-office position
QUALIFICATIONS:
- Detail-oriented, with strong organizational and time management skills
- Ability to work independently and as part of a team in a fast-paced environment
- Strong knowledge of Microsoft 365, including Outlook, Word and Excel.
- Ability to communicate effectively in writing and verbally
- Possess solid problem-solving skills and critical thinking skills
- High school diploma or equivalent required
- Experience in healthcare scheduling; 1-2 years Home Health scheduling experience preferred
- Working knowledge of Wellsky Kinnser preferred
