Responsibilities:
- Work Tuesday - Saturday weekly
- Conduct initial and ongoing assessments of patients' health status and develop individualized care plans
- Administer medication, treatments, and other nursing interventions as prescribed by the patient's physician
- Monitor patients' conditions and provide necessary education and support to help them manage their health
- Document all patient care activities in accordance with company policies and regulations
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for patients
- Communicate regularly with patients, families, and caregivers to provide updates on the patient's progress and address any concerns
Preferred Qualifications:
- Must have current Florida RN license
- Home Health Experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently as well as part of a multidisciplinary team
