Responsibilities
- Under the direction of the Occupational Therapist, assists in evaluation and development of the rehabilitative plan of care and in period re-evaluations as required.
- Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
- Maintain accurate and timely documentation and progress notes.
- Collaborate with other healthcare professionals to ensure patients receive comprehensive care
- Guide and instructs patients in prescribed therapeutic self-care and creative activities that are directed toward improving independence and functioning.
- Attend and participate in department staff meetings and case conference meetings.
Qualifications:
- COTA degree from an accredited program
- A COTA license from applicable state
- One year of home health experience required
- Must have knowledge of Medicare and Medicaid guidelines.
- Current CPR certification
- Valid driver’s license and reliable transportation
- Excellent verbal and written communication skills
- Basic computer skill proficiency. Ability to use Microsoft Windows.
