What are the responsibilities and job description for the Case Manager position at DISC Village?
We are looking for a dedicated Case Manager to support families in our Family Intensive Treatment (FIT) program and fill an immediate opening on our collaborative, mission‑driven team.
Essential Responsibilities:
- Coordinate outreach activities to engage parents, caregivers, and other potential team members in case management and care coordination services.
- Assess immediate and potential crises or safety concerns and develop timely, preventative plans to address identified risks and reduce the likelihood of future crises.
- Develop and maintain comprehensive, family‑centered care plans with measurable goals and actionable steps that reflect family strengths, needs, culture, and priorities, and actively involve the care team in supporting goal achievement.
- Connect parents and caregivers to appropriate services and supports, ensuring coordination and continuity of care across adult and child behavioral health systems, and educate families regarding their parental rights.
- Collaborate with and leverage established community networks, coalitions, and partner organizations to strengthen communication, enhance service coordination, and improve service delivery outcomes.
- Travel as required between offices and service locations, including homes, schools, and community settings where clients receive services.
- Pay: $22.63 hourly
Qualifications:
- Bachelor’s degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related field that includes the study of human behavior and development.
- Minimum of one year of experience working with adults with behavioral health needs and child welfare involvement.
- Bachelor’s or Master’s degree with a major in another field and a minimum of three years of experience working with adults with substance use disorders.
- Must have a valid Florida driver’s license and applicable state-required car insurance.
