Position Overview
The NFT Specialist provides direct support to Medicaid recipients living in nursing facilities who wish to transition back into the community. This role focuses on person-centered planning, community resource coordination, and ongoing follow-along services to support successful transitions and long-term independence.
Key Responsibilities
- Provide direct services to nursing facility residents seeking community-based living.
- Conduct consumer assessments and develop person-centered transition and independent living plans.
- Coordinate all aspects of the transition plan and make appropriate referrals.
- Provide information and referral on community resources.
- Complete outreach to nursing facilities and residents regarding available services.
- Document all consumer interactions within 24 hours and maintain accurate files.
- Submit reports as requested and meet required monthly benchmarks.
- Participate in state and community meetings related to Long-Term Care/NFT.
- Conduct weekly follow-along phone calls and monthly in-person follow-ups per NFT guidelines.
- Meet designated billable unit expectations each month.
- Travel regularly throughout the service area and occasionally outside the area.
Required Competencies
- Knowledge of HIPAA and Mandated Reporter guidelines.
- Familiarity with person-centered processes.
- Knowledge of developmental and mental health disabilities.
- Strong organizational, time management, and problem-solving skills.
- Excellent written and verbal communication.
- Ability to work independently, maintain confidentiality, and respect consumer choice.
- Comfort with public speaking and community outreach.
- Computer proficiency and ability to track timelines.
Qualifications
Candidates must meet ONE of the following:
- Licensed Registered Nurse in Michigan, OR
- Licensed Social Worker in Michigan, OR
- Bachelor’s degree in a health or human services field or Community Health Worker certification and at least 3 years of experience providing health or social services.
Additional Requirements:
- Understanding of the Independent Living Philosophy.
- Three or more years of experience working with community resources.
- Ability to travel to nursing facilities, consumer homes, and community sites, including locations with stairs.
Work Expectations
- Maintain a caseload of 12–20 consumers.
- Cover assigned nursing facilities monthly.
- Ensure all documentation is completed within 24 hours.
- Attend agency meetings, trainings, and collaborative community projects.
- Work a full 32–40 hour week, with occasional flexibility outside typical business hours.
Work Environment & Travel
Work occurs in office settings, consumer homes, community locations, and nursing facilities. Regular local travel and some out-of-area travel are required.
