The Patient Support Coordinator serves as the primary guide for clients from their first visit through connection with ongoing services and resources. This role helps ensure each client feels welcomed, heard, and supported by providing compassionate care, coordinating services, and building meaningful relationships throughout their journey.
Working closely with nurses, patient advocates, and other team members, the Patient Support Coordinator helps bridge medical care with practical support by connecting clients to education, community resources, and Directions programs.
The ideal candidate is compassionate, organized, and able to build trust while helping clients navigate their next steps with confidence.
Pay: $22.00 - $24.00 per hour
