PT- 20-25 Hours/week (Flexible: M-F, Generally 9-5, No Weekends)
$17.00 an hour, plus mileage reimbursed at federal level
Essential Function
Responsible for assisting individuals to become more independent when performing activities of daily living to achieve their valued outcomes and personal goals. Habilitation services are designed to develop skills to increase an individual’s level of independence. Services are primarily community based and revolve around volunteer and recreational opportunities.
Principal Activities
· Work independently and multi-task
· Ensure effective instruction and safety of individual
· Ensure that documentation is kept in billing compliance
· Maintain a safe vehicle and valid driver’s license
· Complete all required trainings in a timely manner
Job Qualifications
· High School Diploma or GED (Required)
· Valid driver’s license with clean driving record (Required)
· Reliable vehicle and current auto insurance (Required)
· Ability to pass background check and drug test (Required)
· 1 year experience working with individuals with disabilities (Preferred)
· Ability to provide services independently in the community
· Excellent written, verbal and organizational skills
· Desire to work with children and adults with special needs
· Willingness to meet each individual’s distinct needs, interests, and aspirations with flexibility and respect
Physical Requirements
· Ability to lift at least 20lbs
· Ability to run at least 500 feet
· Ability to move or push heavy items such as tables
· Must be able to kneel, twist and bend
