Role Description
This is a full-time remote role for a Care Coordinator and Waiver Service Coordinator. The role involves making home visits to provide comprehensive service coordination to individuals in need, including managing care plans, connecting clients with service providers, and advocating for resources tailored to meet individual needs. Daily responsibilities include home visits, maintaining communication with clients and their families, collaborating with healthcare professionals, and ensuring compliance with state and federal regulations. Qualifications
- Expertise in Service Coordination
- Strong Communication and Interpersonal Skills
- Ability to provide exceptional Customer Service
- Knowledge of long-term care services and resources for older adults and individuals with disabilities
- Ability to work independently in a remote setting
- Registered Nurse or Bachelor's degree in social work is required
- Experience in Medicaid-related programs or waiver services is a plus
