1 Schedule Available:
Schedule 1: Monday - Friday 8 AM- 5 PM
On-call Or weekend requirements: Two weekend days per month and 2 on-call shift per month (one on the weekend and one during the week, 5p-10p)
Service Area: Capistrano Beach, Rancho Mission Viejo, Ladera Ranch, San Clemente, and San Juan Capistrano.
Schedule 2: (2 positions) Mon- Fri 8 AM-5 PM
On-call Or weekend requirements: 2-3 weekend shifts per month
13 Week Contract
Service area: Newport Beach, Costa Mesa, Tustin, Irvine, Lake Forest.
Requirements
Must have at least 1 year of Hospice experience within the last 3 years as an RN.
HCHB charting experience is also required.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories, to develop personalized care plans.
- Administer medications via injections, IV infusions, and other routes following strict aseptic techniques and medication protocols.
- Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while adhering to infection control standards.
- Utilize electronic health record (EHR) systems like Epic or Cerner to document patient interactions accurately and efficiently.
- Provide specialized care for individuals with developmental disabilities, memory care needs, or those requiring hospice and palliative medicine support.
- Educate patients and their families on disease management, medication administration, nutrition, and health coaching strategies to promote independence.
- Coordinate with multidisciplinary teams for discharge planning, case management, and ensuring continuity of care across settings such as assisted living facilities or nursing homes.
Qualifications
- Valid RN license with current registration in the state of practice.
- Critical care experience in settings like ICU or emergency medicine is highly preferred.
- Proficiency with EMR/EHR systems such as Epic or Cerner; experience with medical documentation standards including ICD-10/ICD-9 coding is advantageous.
- Strong clinical skills in areas such as cardiac catheterization post-care, dialysis management, ventilator support, and infection control practices.
- Experience working with diverse populations including pediatrics, geriatrics, behavioral health patients, or those with complex chronic conditions like long-term care or trauma medicine.
- Knowledge of HIPAA regulations and confidentiality standards to ensure patient privacy at all times.
- Ability to perform advanced procedures such as phlebotomy, specimen collection/processing, tube feeding management, and physical examinations confidently.
- Excellent communication skills for triage calls, patient education sessions, and interdisciplinary collaboration.
