The Opportunity
As a Care Manager, you’ll play a vital role in connecting Medicaid members with the services they need to thrive—across medical, behavioral health, and social domains. You’ll work in the field, in members’ homes, and in community settings to build trusting relationships, create person-centered care plans, and ensure each individual gets the right care at the right time.
What You’ll Do
- Manage a panel of Medicaid members and deliver proactive, individualized care coordination
- Conduct initial and ongoing assessments to identify clinical, behavioral, and social needs
- Develop and update care and crisis plans with a person-centered, strength-based approach
- Coordinate services including primary care, mental health, housing, recovery support, and public benefits
- Perform field-based visits in homes, hospitals, shelters, or other community locations
- Monitor real-time alerts (e.g., hospitalizations, ED visits, incarcerations) and manage care transitions
- Collaborate with interdisciplinary teams and external providers to align care
- Accurately document all member interactions in the Electronic Health Record (EHR)
- Support wellness education and chronic disease self-management
- Help members navigate public systems including Medicaid, SNAP, housing, and legal services
- Participate in team huddles, case conferences, and continuous quality improvement initiatives
What You Bring
Education
- Associate’s degree with 3 years of relevant experience, OR
- Bachelor’s degree in human services field (social work, public health, psychology, etc.) with 1 year of relevant experience
Experience
- Field-based care coordination or case management experience strongly preferred
- Health Home, behavioral health or managed care exposure is a plus
Skills
- Empathetic, person-centered approach to care
- Excellent documentation, organizational, and time management skills
- Proficient in Electronic Health Records and Microsoft Office
- Ability to work independently and travel throughout the community
- Bilingual abilities are highly desirable
