The ideal candidate will have experience in a hospital or home community care setting, and will possess strong physiology knowledge. Provide direct patient care for a male client in his home, including administering medications, taking vital signs, and assisting with daily living activities.
Develop and implement individualized care plans, ensuring that patients receive the most effective and efficient care possible.
Collaborate with other healthcare professionals, including physicians and nurses, to ensure that patients receive comprehensive care.
Utilize electronic medical records (EMRs) to document patient care and communicate with healthcare professionals.
WellSky EMR experience a plus, but wil train.
Maintain accurate records of patient care, including progress notes and discharge summaries.
Experience working in a hospital or home community care setting.
Strong physiology knowledge and experience with airway management.
Ability to travel to patients' homes as needed.
Ability to work effectively in a fast-paced environment, with a focus on providing high-quality patient care.
