Part time
Experience: 1 to 2 years preferred.
Licenses required: SSW or LCSW
Location: Utah County

Medical Social Worker Responsibilities:
- Provide supportive counsel aligning to the patient’s and family needs
- Assisting patients and their families to understand, accept and follow medical recommendations
- Help utilize community resources available for the patient and their family
- Maintains collaborative relationships with organization personnel to support patient care
- Assists physician and other team members with understanding significant social and emotional factors related to the patient’s health
- Help integrate clinical guidelines, protocols and other metrics in development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of care
- Maintain clinical records on all patients referred to social work
- Other duties as delegated by the Clinical Director/Supervisor
Job Type: Part-time to full time
Pay: $19.00 - $25.00 per hour
