Pay Range
$28.10 - $41.80 /hour

- Conducts in-depth assessments/reassessments covering psycho-social, rehabilitation and environmental concerns
- Refers clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional level
- Develops the care plan, implement the services and conduct follow-up and monitoring of the plan
- Identifies and develops support systems for the client
- Consults with the Health Practitioner
- Performs case recording and reporting
Job Requirements
Minimum Qualifications:
- Bachelors Other in social work,psychology, counseling, rehabilitation,gerontology or sociology, One (1) year experience working with theelderly and individuals with disabilities or
- Two years of experience providing social service in one or more of the fields of aging health and long term care services required
- Ability to relate to clients and families
- Excellent communication and presentation skills
