What You Will Do as a Hospice Account Executive

As a Hospice Account Executive, you will match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise to meet/exceed census and goals. Some of your primary responsibilities will include:
- Communicating with the Interdisciplinary team daily to ascertain information and knowledge about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events
- Managing the electronic referral system for assigned centers and develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
- Completing clinical reviews via Allscripts and/or onsite evaluation as needed or requested, and coordinating a seamless transition to assigned centers
- Remaining visible as assigned hospital and being available as a resource to promote Stonerise in a positive manner
- Abiding to all HIPPA laws and observing confidentiality policy
- Presenting the benefits of long and short-term skilled nursing at Stonerise to patients, families, hospital employees, etc.
