Lead Case Manager (Inglewood Adult Services)

As a Lead Case Manager (internally referred to as a Lead Care Manager), you will build meaningful relationships, coordinate whole-person care, and serve as a trusted advocate for members as they navigate their recovery and wellness journey.

What You'll Do
Working collaboratively with healthcare providers, hospitals, community organizations, and support networks, you will coordinate seamless, whole-person care that helps members achieve their goals. This role is highly community-focused, requiring you to meet members where they are by providing services in homes, shelters, clinics, and other accessible settings.
You will:
- Conduct comprehensive assessments and develop personalized care plans that address the full spectrum of member needs, including physical health, behavioral health, substance use, housing stability, and access to social services.
- Advocate on behalf of members, connecting them to essential resources such as benefits, transportation, housing assistance, and community-based support.
- Use motivational interviewing, trauma-informed care, and harm-reduction practices to empower individuals to engage in their care, improve their well-being, and build long-term stability and independence.
What You'll Bring
- High school diploma or GED required; bachelor's degree in social work, psychology, counseling, nursing, or a related field preferred.
- Two or more years of experience supporting Medi-Cal members, behavioral health populations, or individuals with complex care needs.
- Strong relationship-building and communication skills with the ability to engage individuals from diverse backgrounds.
- Knowledge of community resources, care coordination, trauma-informed care, motivational interviewing, and harm-reduction practices.
- Current California driver's license, reliable transportation, and the ability to travel throughout the South Los Angeles community.
- Strong organizational, critical-thinking, and documentation skills with the ability to manage multiple priorities effectively.
