Overview
Keystone Bluffs is an Assisted Living facility that started in 2001 located in Duluth, MN. Keystone Bluffs offers 80 beautiful apartments designed with single-level living in mind. We are committed to providing the utmost quality of care, emphasizing person-centered care planning. We believe our nursing staff is the foundation of our success. This role would typically work from 8:00AM - 4:30PM Monday through Friday with some flexibility, and it is expected that this person would participate in an on-call rotation. This role would require flexibility in covering weekends when supervisors are on vacation.
Job Summary
The RN Case Manager will support the Director of Nursing by overseeing clinical staff, ensuring regulatory compliance, and managing all aspects of resident care. This role involves case managing residents, coordinating care plans overseeing assessments, and serving as a key liaison between residents, families, and health care providers.
Responsibilities
- Conduct timely assessment ( 14-day, 90-day, annual ) and Mini-Mental evaluations every 90 days.
- Monitor and address changes in condition, including vitals, labs, weight, and mental status.
- Coordinate and attend care conferences; update families regularly.
- Review and follow upon nursing communications and resident concerns.
- Process physician orders and ensure all documentation, follow-up, and staff training are completed.
- Ensure treatment plans, service agreements, and care plans are current and reflect resident needs.
- Supervise and support nursing staff, ensuring availability and proper delegation of tasks.
- Maintain compliance with all MN State, Federal, and Assisted Living regulations.
- Participate in weekly on-call rotation and provide in-person support as needed.
- Track and perform required supervisory visits and chart audits.
- Communicate effectively with physicians, home care agencies, and discharge planners.
- Address and resolve medication errors; ensure timely reporting and intervention,
- Promote strong relationships with residents, families, and staff.
- Maintain organized, accurate records and ensure readiness for audits and surveys.
Qualifications
Possess a valid Minnesota RN License
Minimum of 2 year experience
