What are the responsibilities and job description for the Behavioral Health Case Manager position at Diamond Peak Health Care?
Job description:
Who We Are: A Different Kind of Clinical Family
Let’s be honest—our team is different, and we love it that way! Bella-Nacole Mental Health Services and Diamond Peak Health Care work hand-in-hand with Idaho Medicaid and the Department of Behavioral Health to provide stable housing and life-changing mental health services for Idaho’s most chronically mentally ill. Our members have diagnoses like schizophrenia, bipolar, PTSD, etc.
We joke, we laugh, we tease, and we genuinely enjoy working together. We are highly professional with excellent boundaries, but we also know how to let loose and have fun. If you want a rigid, stuffy corporate job, this isn't it. At the HART (Housing for Adult Residential Treatment) Program, we are a family!
About the HART Program
The HART program is a specialized, intensive program designed for Idaho’s Severe and Persistent Mental Illness (SPMI) population. By combining therapy, CBRS, case management, primary care, and medication management under one roof, we provide an unprecedented level of coordinated care. Our goal is simple but profound: aid members in achieving stability, maintaining autonomy, and staying out of hospitals and jail.
What You’ll Do (The Role)
As a House Case Manager at Bella Nacole, you serve as a vital lead within our high-touch HART wraparound program, providing dedicated care management for up to 16 residents. In this role, you act as the central connector for each resident’s overall well-being by proactively scheduling medical, psychiatric, and specialty appointments, following up on care instructions, and ensuring all treatment recommendations are fully carried out. You maintain continuous alignment across our multidisciplinary team, keeping internal staff and outside providers closely coordinated so that every resident receives comprehensive, seamless support across all functional areas.
In addition to managing day-to-day healthcare logistics, you actively bridge the gap between residents and the broader community by establishing strong linkages to local resources, vocational programs, and essential social services. You serve as a primary liaison with insurance companies, Social Security, and other government agencies to help residents apply for, secure, and maintain critical benefits such as Medicaid, SSI/SSDI, and housing support without interruption. Through detailed documentation, ongoing benefit monitoring, and persistent advocacy, you ensure that every resident has the stable framework and community support necessary to thrive within our residential program.
