What are the responsibilities and job description for the Case Manager II - Family Preservation (Lehigh County - PA) position at Diakon?

General Responsibilities:
- Responsible for serving as a resource person, case manager, counselor, and trainer to support the youth and families of the Diversion Truancy program.
- Maintains a caseload of five (5) youth and families.
- Establishes and maintains professional and supportive relationships with youth, birth families, referring personnel, treatment providers, school districts and others involved in the lives of children and youth.
- Schedules, completes paperwork necessary (ISP), and participates in initial Treatment Team meetings and quarterly reviews.
Education/Training: A BA or BS degree in social service or a related field of study normally required.
Experience: A minimum of one (2) year’s experience in a related child welfare or human service position normally required.
ADA: ability to bend, stoop, sit, stand, reach and lift items weighing up to 50 lbs.; valid driver's license; visual and auditory skills.
