RN Case Manager- FT- Days- Resource Management

Summary:

The Case Management Supervisor manages the performance of ALL CM duties. Assists with the management of discharge planning from the beginning of the admission through to transition to next level of care. Coordinate all CM DHR campus staff on work schedules and operations of the department. Responds to the daily operations of the departments; maintains a staffing plan that meets organizational needs. Works with Director with the hiring, firing, orientation, training and evaluation according to established guidelines. 

POSITION EDUCATION/ QUALIFICATIONS­:

• Bachelor’s Degree Nursing preferred.
• Registered Nurse in the State of Texas required.
• Computer skills required with knowledge of Microsoft Office suite.
• Good written and verbal communication skills required.
• Some evening or weekend work will be required
• Bilingual – English/Spanish

Responsibilities:

• Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices.
• Maintain professional relationships with organizations, county personnel, families and service providers.
• Assist in the hiring and evaluation of all Case Managers.
• Assist in identifying training needs for the Case Management program. Coordinate training for staff.

• Help Case Managers develop mediation and intervention strategies and implement them to resolve conflicts.

• Act as liaison between Case Managers and Board.
• Other duties as assigned. 

Other information:

JOB KNOWLEDGE/EXPERIENCE­:

• Five (5) years’ experience, two (2) of those years in supervisory positions.
• Knowledgeable in case management principles, procedures and practices.
• Knowledgeable in crisis intervention principles and practices.
• Good personal relations skills
• Demonstrates competent computer skills, including use of assigned word processing Program
• Ability to present ideas effectively, in both oral and / or written form.
• Ability to conduct detailed analytical evaluations and studies and prepare related reports and

• recommendations.

• Skill in establishing a balanced perspective on mission effectiveness and business results is critical.
• Knowledge of State, Federal, and JCAHO regulations.

CUSTOMER SERVICE:

Provide excellent customer service to all DHR customers. All employees are required to attend the DHR C.A.R.E.S program which outlines the Customer Service Principals including: Commitment, Accountability, Respect, Excellence and Service.

AGE SPECIFIC:

Employees must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in his/her assigned unit. The individual must demonstrate knowledge of principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirement relative to his or her age.
