Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team, providing exceptional care to patients in their final stages of life. The ideal candidate will possess a strong background in nursing with experience in hospice and palliative medicine, ensuring that patients receive the highest quality of care tailored to their individual needs. This role requires a deep understanding of patient assessment, medication administration, and emotional support for both patients and their families.
Responsibilities
- Deliver comprehensive nursing care to hospice patients, focusing on comfort and quality of life.
- Conduct thorough patient assessments, including vital signs, medical history, and physical examinations.
- Administer medications and treatments as prescribed, including IV infusion and pain management techniques.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Provide emotional support and counseling to patients and families during end-of-life discussions.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Educate patients and families on disease processes, treatment options, and end-of-life care.
- Participate in case management activities to coordinate care across various settings.
- Ensure adherence to CHAP and CMS standards and best practices in hospice care.
Experience
Candidates should have a robust nursing background with experience in the following areas considered beneficial:
- Hospice & palliative medicine
- Long-term care experience
- Acute care settings (ICU, emergency room)
- Medical office experience or home care
- Familiarity with EMR/EHR systems
- Experience working with individuals with developmental disabilities or memory care needs
- Skills in acute pain management, catheterization, phlebotomy, and IV insertion
- Experience with geriatric populations and senior care
The ideal candidate will also demonstrate proficiency in critical thinking, effective communication skills, and a commitment to providing compassionate patient-centered care.
Pay: $ $60.00 per hour
Expected hours: No less than 10 per week
