What are the responsibilities and job description for the Case Manager - State General Funds position at DEVELOPMENTAL PATHWAYS INC?
Developmental Pathways (DP) has an opportunity for a Case Manager (CM) – State General Funds (SGF) in our Case Management department. The CM – SGF will manage a caseload of individuals receiving State General Funds for services and maintain documentation needed quarterly and annually. The CM – SGF will make collateral contacts with such community resources, such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure their assistance. The CM – SGF will ensure the accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to providers, individuals, and family members.

Schedule: Enjoy a 4-day, 40-hour workweek (Monday–Thursday) with the flexibility to work remotely, in-office, and in the field.
This position requires in-person presence to support essential responsibilities such as annual meetings, quarterly mentorings, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Requirements

Bachelor’s Degree; or
-
Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities; or
-
A combination of education and relevant experience appropriate to the requirements of the position

Valid Colorado motor vehicle license and provide proof of motor vehicle insurance

Ability to meet our driving requirements - no suspensions/ no more than two tickets in the past three year
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.
Preferred

One (1) year of experience in developmental disabilities/long-term services and support

Additional Responsibilities

Manages a caseload of individuals receiving State General Funds for services and maintains documentation needed quarterly and annually

Researches, requests, and obtains potential services and/or providers to meet individual needs/preferences as determined in the SP

Complete required documentation related to monitoring activities, allocation/check requests, ensuring the effectiveness of services delivered (as authorized by the service plan) and ensuring identified outcomes are met, advocates for individual service needs

Facilitate Member Identified Team (MIT) meetings to coordinate services to meet individual's needs; ensure appropriate

Complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools

Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary team members

Provide case management services and support to temporarily assigned individuals; support with department and team coverage as needed
