About the Role
DDRC is seeking a Licensed Practical Nurse (LPN) to provide nursing care and coordination for individuals in community-based settings. This role supports person-centered care, works closely with interdisciplinary teams, and ensures high-quality, compliant healthcare services outside of a traditional hospital environment.
What You'll Do
In this role, you'll provide nursing care and coordinate health services to support individuals in community-based settings. You'll partner with care teams, medical providers, and families to ensure quality care, accurate documentation, and regulatory compliance.
- Deliver nursing care, including assessments, medication coordination, and physician order implementation
- Coordinate care and communicate with providers, teams, and families
- Maintain accurate, compliant medical records
- Act as liaison with physicians, pharmacies, and other providers
- Monitor health changes and coordinate follow-up care
- Ensure timely completion of exams, consents, and orders
- Train and guide staff on medical procedures and infection control
- Support quality reviews and regulatory compliance
- Participate in on-call rotation
- Mentor and onboard new nursing staff
- Provide specialized support (e.g., G-tube care)
- Lead or assist with trainings, committees, and projects

What We're Looking For
- Active LPN license in Colorado (or ability to obtain)
- Knowledge of medication administration and nursing standards
- Experience in developmental disabilities, home health, or similar settings preferred
- Strong communication and documentation skills
- Ability to work independently and with a team
