Position Summary:

This position will work under the direction of a registered nurse while assisting patients account to care plans and department policy. This role ensures a comprehensive team approach in the care of patients and serves as liaison between nursing staff and patient in the performance of care plans.

ESSENTIAL FUNCTIONS:

  • Assesses patient physical, psychological, developmental, cultural and discharge planning needs. Reviews patient history with patient/family and assures completion within appropriate timeframe. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
  • Formulates a plan of care utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient need.
  • Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements.
  • Evaluates the patient's response to care and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members.
  • Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment. Notes physician orders accurately and in a timely manner.
  • Provides care based on the best evidence available and may participate in research activities within clinical practice.
  • Perform other functions as assigned by the Director of Intake or the Director of Nursing.
  • Coordinates physician to physician acceptance calls, participates in and documents acceptance calls
  • Assists with determining patient benefits/eligibility
  • Assists the team in proactively managing requests for admission and facilitates throughput of patients
  • Accurately collects data required for intake of patients from a variety of locations

SUPERVISORY RESPONSIBILITY:

This position has no supervisory or managerial responsibilities.

  • Ability to perform tasks independently.

WORK ENVIRONMENT:

This job operates in a clinical setting. This role routinely requires interaction with patients, documentation, and utilization of standard office equipment.

PHYSICAL DEMANDS:

  • Ability to exercise self-control in potentially volatile situations such as being verbally or physically confronted in a threatening or aggressive manner; must be able to work and concentrate amidst distractions such as noise, conversation and foot traffic; ability to handle interruptions often and be able to move from one task to another; must be flexible and not easily frustrated in dealing with differences of opinions.
  • Ability to stoop, kneel, crouch, crawl, reach, stand, walk, push, pull, lift, grasp, and be able to perceive the attributes of objects such as size, shape, temperature, and/or texture by touching with skin, particularly that of the fingertips. Ability to express and exchange ideas via spoken work during activities in which they must convey detail or important spoken instructions to others accurately, sometimes quickly and loudly. Ability to express and exchange ideas via written assignments effectively and accurately. Hearing to perceive the nature of sound with no less than 40 db loss @ Hz, 1000 Hz, and 2000 Hz with or without correction; ability to perceive detailed information through oral communication and to make fine discriminations in sound. Performs repetitive motions with wrists, hands and fingers.
  • Worker is not substantially exposed to adverse environmental conditions; conditions are typical of office settings and administrative work.
  • Must be able to address and prioritize multiple task demands within established time frames.

REQUIRED EDUCATION / EXPERIENCE:

  • Must possess a current Arizona RN license.
  • BLS certification is required.
  • Associate Degree in Nursing (ADN) or Diploma nurse is required. BSN is preferred.
  • Two years behavioral health experience required.
  • Prior Intake experience is a plus
