Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patients’ vital signs, health status, and behavioral changes with attention to HIPAA privacy standards
- Support medication administration following care plans and safety protocols
- Provide companionship and emotional support to promote mental well-being
- Prepare nutritious meals and assist with meal planning tailored to individual dietary needs
- Maintain a clean, safe environment through regular cleaning and sanitation tasks
- Use specialized equipment such as Hoyer lifts or resident’s
to ensure safe transfers and mobility assistance - Observe patient conditions closely, report concerns promptly, and collaborate with healthcare teams for optimal care
- Support clients in community outings or activities to enhance social engagement
- Follow all safety procedures including CPR, first aid, and emergency response protocols
Skills
- Proven experience working in assisted living, home health, nursing homes, or group homes with individuals who have developmental disabilities or dementia
- Knowledge of Alzheimer’s care, memory care, autism support, hospice care, and long-term care environments
- Strong patient observation skills with the ability to monitor vital signs and assess behavioral changes accurately
- Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or wheelchairs
- Excellent communication skills for working effectively with clients, families, and healthcare professionals
- Familiarity with HIPAA regulations ensuring confidentiality of patient information
- Experience in medication administration, care planning, and documentation practices
- Compassionate approach combined with patience when assisting individuals with disabilities or cognitive impairments
- Basic cooking skills for meal preparation aligned with dietary guidelines
