What are the responsibilities and job description for the RN, Case Manager position at Desert Oasis Healthcare?
The RN Case Manager is the member of the interdisciplinary team who is the pivotal person in identifying the physical, psychological, social and spiritual needs of the patient and family. The RN Case Manager initiates appropriate intervention and support for the patient and family upon admission and provides a continuously appropriate, comprehensive and responsive plan of care.
- Professional Duties- Coordinates the patient’s and families care and services including those of the team, social worker, chaplain, home health aide, volunteer and physician.
- - Responds necessary information to the Team Manager.
- - Monitors the re-certification process.
- - Participates in Interdisciplinary Team Meetings as indicated.
- - Documentation is complete, accurate and timely.
- - Ensures and documents appropriate use of the medications, durable medical equipment and supplies.
- - Educates patients, families and other caregivers.
- - Provides skilled nursing as determined by the interdisciplinary plan of care.
- - Updates care plan regularly and integrates plan of care with any new changes in patient condition.
- - Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Dietary, Speech and Pharmacy.
- - Performs on site evaluations of Certified Home Health Aides (CHHA) and volunteers.
- - Participates in the on-call rotation, weekends and holiday schedules if situation arises.
- - Integrates Quality Improvement results into the established plan of care.
- - Participates in the orientation of new team members and / or act as a preceptor to nursing students as assigned by the Team Manager.
- - Attends regularly scheduled in-services and educational conferences.
- - Reconcile medication with each change of level of care and update the electronic health record and Hospice pharmacy.

- Qualifications- Current California RN License
- - 1 year experience as a professional nurse, with Diploma, Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN), BSN may substitute for 1 year of work experience. Previous experience in hospice or home health, preferred.
- - Knowledge of the principles and practices of primary medical and nursing care.
- - Ability to communicate effectively with individuals within all levels of the organization, patients, providers, and others.
- - Ability to work independently. Ability to utilize critical thinking and judgement to problem solve.
- - BLS required within 60 days of hire.
- - Valid California’s Driver’s license and proof of auto insurance.
