Care Specialist

About Us:
Desert Hearts Nursing and Home Care is dedicated to providing compassionate, high-quality care to veterans and seniors in the comfort of their homes. We are growing and looking for a detail-oriented, reliable team member to support our scheduling and care coordination team.

Position Overview:
We are seeking a highly organized and professional Care Specialist to manage and coordinate care for our veteran clients. This role requires strong multitasking skills, attention to detail, and the ability to adapt quickly in a fast-paced environment.

Schedule:

• Monday: 8:00 AM – 1:00 PM
• Tuesday – Friday: 12:00 PM – 5:00 PM
• On-call: 7 days per month (may include 1–2 full weeks per month or consecutive days depending on rotation)

Key Responsibilities:

• Coordinate and manage schedules for veteran clients and caregivers
• Adjust schedules in real-time to ensure continuity of care
• Communicate with caregivers, clients, and leadership team
• Collaborate with office staff, including the Director of Care Operations
• Maintain accurate and timely documentation
• Handle high call volume and urgent staffing needs
• Assist with last-minute shift coverage when needed

Qualifications:

• Scheduling or healthcare background preferred
• Strong organizational and multitasking skills
• Ability to work in a fast-paced, sometimes high-pressure environment
• Detail-oriented with strong documentation skills
• Professional communication and problem-solving abilities
• Reliable, flexible, and team-oriented
